For your convenience, Club Centaur Rewards now provides you with a Win/(Loss) Statement that reflects your gaming and racing activity at Hoosier Park Racing & Casino, Indiana Grand Racing & Casino and any of our Off Track Betting locations where Club Centaur Rewards is available, including the downtown Indianapolis Winner’s Circle and the Merrillville and Fort Wayne OTBs. This information will be available for the 2016 tax year on January 15, 2017.
Below are three easy ways to request a Win/(Loss) Statement for your gaming and racing activity:
- Simply fill out the online request form below to complete.
- Visit a Club Centaur Rewards center at either Hoosier Park or Indiana Grand with a valid ID and get your Win/(Loss) Statement
- Download the Win/(Loss) Statement Request Form below, complete and mail it to your property of choice. Upon receiving the request, your Win/(Loss) statement will be sent to you within 7 – 10 business days.
Win/(Loss) Statement Request Forms are also available at Cage Cashier Windows at both Hoosier Park and Indiana Grand.
ONLINE WIN/(LOSS) STATEMENT REQUEST FORM
Your Win/(Loss) Statement will include estimated Electronic Games or Horse Wagering win/(loss) information from Indiana Grand Racing & Casino and Hoosier Park Racing & Casino. Wagering actual win/(loss) for Indiana Grand is as of May 6, 2016. The tracking system used in providing this information is based on the use of your Club Centaur card. Therefore, this statement will not reflect an accurate accounting record – it merely provides an estimate you can use to compare your records. The IRS recommends that you keep your own records of your gaming activity.
